肝细胞癌根治性切除术后早期肝内复发的影响因素分析

目的探讨影响肝细胞癌(HCC)根治性切除术后早期肝内复发的独立危险因素,为预测术后早期肝内复发提供理论依据。方法 70例HCC患者,行根治性肝癌切除术后早期复发(<2年)40例,非早期复发(>2年)30例,对两组临床资料进行分析。结果单因素分析表明:入院主诉为腹痛、术前血清ALP>135 U/L、术前血清AFP>400 ng/ml、术中输血及肿瘤直径≥5 cm与HCC术后早期复发相关(P<0.05)。多因素分析表明腹痛、术前血清ALP及AFP水平、术中输血、肿瘤直径是影响HCC术后早期肝内复发的独立危险因素(P<0.05)。结论 HCC术后早期肝内复发是多因素共同作用的结果,其中腹痛、术前ALP及AFP水平、术中输血及肿瘤最大直径是影响HCC术后早期肝内复发的独立危险因素。

Analysis of Factors Influencing Early Intrahepatic Recurrence of Hepatocellular Carcinoma After Curative Resection

Objective To investigate the risk factors influencing early intrahepatic recurrence of hepatocellular carcinoma(HCC) after curative resection,so as to provide the reference for forecasting the early intrahepatic recurrence.Methods Seventy patients who underwent curative resection for HCC were divided into two groups:40 cases in early recurrence group(<2 years) and 30 cases in non-early-recurrence group(>2 years),then the clinical data of two groups were analyzed.Results Univariate analysis showed that the symptoms of abdominal pain,preoperative serum ALP>135 U/L,preoperative serum AFP>400 ng/ml,perioperative blood transfusion and tumor diameter ≥5 cm might correlate with the early intrahepatic recurrence of HCC after curative resection(P<0.05).The result of multivariate analysis indicated that the symptoms of abdominal pain,preoperative serum ALP and AFP,perioperative blood transfusion,and tumor size were the independent risk factors influencing postoperative early intrahepatic recurrence(P<0.05).Conclusion Early intrahepatic recurrence after curative resection for HCC attributes to lots of factors.The symptoms of abdominal pain,serum AFP and ALP,perioperative blood transfusion,maximum tumor diameter are the independent risk factors of early intrahepatic recurrence in HCC patients after curative resection.
