摘    要:目的探讨螺旋CT双期增强扫描对小肾癌的诊断价值。方法对12例临床考虑小肾癌患者(男11例,女1例,年龄40~65岁)行螺旋CT双期增强扫描,观察比较肿瘤在肾皮质期、髓质期的增强情况,并与手术病理比较。结果皮质期11例肿瘤显著强化,1例呈轻度强化;实质期12例均显示强化迅速减退,呈快进快退表现。与病理对照,SCT术前正确诊断11例小。肾癌,另1例误诊为良性腺瘤。结论小肾癌的双时相螺旋CT增强扫描的增强特点对其诊断具有重要意义。该技术可在小肾癌的检查中作为常规方法使用。

Abstract:Objective To evaluate the usetulness of dual-phasic spiral CT scanning in chagnoslng small renal cell carcinoma (SRCC).Methods Twelve patients (11 males and 1 femal, aged 40 to 65 years)with clinically considered SRCC underwent dual-phasic spiral CT scanning.The enhanced patterns of SRCC lesions during the cortical phase and the parenehymal phase were dynamically observed and compared with pathologic findings.Results Among 12 patients with SRCC, 11 cases appeared obvious enhancement and 1 case appeared slight enhancement during the cortical phase,while all 12 cases appeared rapid decline of the initial enhancement during the parenchymal phase.Comoared with postoperative pathology,preoperative spiral CT scanning correctly diagnosed 11 cases of SRCC,while only 1 case was misdiagnosed as benign adenoma.Conclusion During dual-phasic spiral CT scanning,SRCC lesions have certain enhanced feature which is helpful for the diagnosis and therefore this technique should be taken as a conventional modality to detect SRCC.
