重症急性胰腺炎时血浆内毒素和肿瘤坏死因子-α水平变化及生长抑素的治疗作用

目的 观察生长抑素(施他宁)对重症急性胰腺炎的治疗效果及其对重症急性胰腺炎患者血浆内毒素和TNF-α水平的影响。方法62例重症急性胰腺炎患者随机分为治疗组和对照组，治疗组采用施他宁治疗，对照组给予一般常规治疗，于第l、3、5、7天抽血测定血浆内毒素和TNF-α。结果重症急性胰腺炎患者入院第l天的血浆内毒素和TNF-α水平明显高于健康人；治疗组患者血浆内毒素和TNF-α水平下降速度明显快于对照组。结论 重症急性胰腺炎患者早期均存在不同程度的内毒素血症及血浆TNF-α水平升高，生长抑素治疗重症急性胰腺炎疗效肯定，治疗组患者血浆内毒素和TNF-α水平下降速度均明显快于对照组。

The efficacy of growth inhibitors in the treatment of severe acute pancreatitis and its effect on plasma endotoxin and TNF-alpha

Objective To evaluate the efficacy of somatostatin in the treatment of severe acute pancreatitis and its effect on plasma endotoxin and TNF-alpha. Methods 62 cases with severe acute pancreatitis were divided into treatment group and control group. The treatment group was treated with somatostatin, while control group only accepted routine management. Plasma endotoxin and TNF-alpha were examined on 1,3,5,7 days.Results The first day on admission, plasma endotoxin and TNF-alpha in the treatment group were remarkably increased compared with that of healthy subject. Endotoxin and TNF-alpha in the treatment group decreased more rapidly than in the control group. Conclusion In the early stage of severe acute pancreatitis, there exist different endotoximia and increased plasma TNF-alpha. In view of this, somatostatin is surely efficient in treating severe acute pancreatitis, in which endotoxin and TNF-alpha level decrease more rapidly in the treatment group than in the control group.
