600例胆石症的超声诊断与误诊原因分析

目的 回顾分析我院15年来600例手术证实的胆石症术前超声诊断，总结漏误诊经验。方法 超声诊断和手术资料分析。结果 术前超声诊断胆石症正确率97．8％（587例），漏误诊率2．2％（13例）；漏误诊的胆总管结石7例，占106例胆总管结石的6．6％。结论 超声诊断胆石症的敏感性和特异性均好，但诊断胆总管结石有局限性。

Ultrasonographic examination and missed or confused diagnoses in 600 cases of cholelithiasis

Objective This article reports the diagnoses of ultrasonographic examination in 600 cases of cholelithiasis operated in our hospital during last 15 years,and analyses retrospectively the causes of missed or confused diagnoses with sonography.Methods To process data of the sonographic examination and operation.Results The accuracy of sonographic diagnosis is 97.8%(in 587 cases).The erroneous diagnoses occurred in 13 cases (2.2%).In 106 cases of choledocholithiasis,there were 7 cases of misdiagnosis (6.6%). Conclusion Our material confirms that songraphy is a highl senstive and specific modality for the diagnosis of stones in the gallbladder,but it is distinctly less powerful for the detection of stones in the common bile duct.
