胸膜外脂肪的MRI及CT对照研究

摘    要:目的 研究MRI和CT对正常及异常胸膜外脂肪显示的价值。方法对21例正常人行常规和薄层CT,以及MRI检查,采用了T1WI、T2WI、化学位移同、反相位成像、STIR序列扫描。计算MRI、常规CT及薄层CT显示胸膜外脂肪的敏感度,并由两位有经验的影像诊断医师进行对比观察和评分。并对30例累及胸膜外脂肪的病人进行CT和MRI 扫描。数据用SPSS 9.0软件包处理,用pearson χ2检验。结果 21例中,16例在MRI上见到胸膜外脂肪,敏感度为76.19%;常规CT可显示7例胸膜外脂肪,敏感度为33.33%;薄层CT可显示14例胸膜外脂肪,敏感度为66.67%。。MRI与常规CT比较,χ2=7.785,P值=0.005,MRI与薄层CT比较,χ=0.467,P=0.495。在MRI对常规CR的比较中T值均为0,P<0.005。在MRI对薄层CT的比较中,医师A所得的T=+10,P<0.005;医师B所得的T=16.5亦P<0.005。MRI及CT可以观察到胸膜外脂肪的移位、受压、中断、增厚以及信号/密度改变。结论 在正常人,MRI对EPF的显示优于CT。通过不同成像序列图像的相互对比,提高了对于EPF的认识,为胸膜周围病变的诊断提供了一种可行而有用的方法。

Abstract:Purpose To investigate the visualization of normal andabnormal appearance of the extrapleural fat(EPF) on MRI and CT.Materials and Methods The axial MR scan of 21 healthy peoplewere taken, using the T1WI,T2WI,RF-FAST in-phase, opposed-phase and STIR sequence on a 1.5T MR scanner (Marconi Eclipse).The MR imaging was compared with regular and thin-slice CTscan. The sensitivity of EPF on MRI , regular and thin-sectionCT were calculated. Two experienced radiologists did the scorethose images to evaluate the ability of the reveals of EPF. 30 casesof different diseases involving EPF were studied with those MRsequences and CT scan. Results The sensitivity of EPF on MR,regular CT and thin-slice CT is 76.19%, 33.33%, and 66.67%respectively. The sensitive of MR imaging looked better than thatof regular CT (P<0.005), but no significant difference between theMR and thin-slice CT images. The films were evaluated by tworadiologists. The conclusion was that MR imaging superior toregular CT in display of EPF (P<0.005). MR imaging and CT scancan reveal the changes of shiftiness, compression, interruption andthickening of EPF, and the signal or density change of EPF.Conclusion MRI displays the EPF is superior to CT scan. We canapply all those sequences together, and get much more informationof EPF. It is a helpful method for the diseases involving EPF.[
