乳腺导管造影对乳腺导管内病变良、恶性的诊断

目的 为早期明确诊断乳腺导管内病变提供客观依据。方法 对355临床表现为乳头溢液，经病理证实的乳腺肿瘤的导管造影表现进行回顾性分析、归纳、总结。结果 355例临床表现为乳头溢液的乳腺肿瘤（良性肿瘤334例，乳腺癌21例）的导管造影表现：226例分布在主导管及一级分支导管（良性肿瘤221例，乳腺癌5例），129例分布在二、三级及小分支导管（良性肿瘤113例，乳腺癌16例），348例导管内充盈缺损（良性肿瘤334例，乳腺癌14例）；86例导管中断（良性肿瘤76例，乳腺癌11例），131例导管壁破坏（良性肿瘤117例，乳腺癌14例），173例导管扩张（良性肿瘤168例，乳腺癌5例），6例导管狭窄（良性肿瘤3例，乳腺癌3例）；乳腺良性肿瘤和乳腺癌在肿瘤发生导管部位、充盈缺损形态、导管中断、导管壁破坏上具有统计学差异。乳头状瘤、乳头状瘤病和乳腺癌导管造影诊断的灵敏度分别为80.1%、76.2%、76.2%。 结论 乳腺导管造影对鉴别导管内良恶性肿瘤具有重要价值。

The diagnostic value of galactography in breast tumor with nipple discharge

【Abstract】 Objective To analyze the diagnostic value of galactography in identifying benign tumors and breast cancer with nipple discharge，and provide information for clinic. Methods The galactographic features of 355 breast tumors with nipple discharge proved by pathology were analyzed retrospectively. Results The galactographic features of 355 breast tumors (334 benign tumors, 21 breast cancer): 226 cases were located in main mammary ducts(221 benign tumors, 5 breast cancer), 129 cases were located in subsegmental duct and its branchesand(113 benign tumors, 16 breast cancer), 348 cases presenting intradutal defect(334 benign tumors, 14 breast cancer), 86 tumor cases presenting ductal obstruction(76 benign tumors, 11 breast cancer), 131 tumor cases presenting ductal wall breakage(117 benign tumors, 14 breast cancer), 173 tumor cases presenting ductal dilatation(168 benign tumors, 5 breast cancer),.6 tumor cases presenting ductal stenosis( 3 benign tumors, 3 breast cancer). Significant different of tumor location、intradutal defect、ductal obstruction、ductal wall breakage were found in benign tumor and breast cancer.The accuracy rates of galactography for intraductal papilloma、paoillomatosis and breast cancer with nipple discharge were 80.1%、76.2%、76.2%. Conclusion galactography are very helpful for differentiating between benign tumors and breast cancer with nipple diacharge.
