慢性充血性心力衰竭患者血浆脑钠素、心钠素浓度与心功能的关系

目的观察慢性充血性心力衰竭（CHF）患者血浆脑钠素（BNP）和心钠素（ANP）浓度与心功能之间的关系，探讨其临床意义。方法75例CHF患者为实验组，按NYHA分级分为四个亚组，另选21例健康者为对照组，采用放免法测定各组血浆BNP和ANP浓度，用超声心动仪（SONO1000）测定左室射血分数（LVEF）。结果CHF组血浆BNP浓度显著高于对照组（P〈0.01），且心功能越差升高越明显；血浆BNP浓度与LVEF之间具有良好的负相关（r=-0．59，P〈0.01）。结论CHF患者血浆BNP、ANP浓度显著升高，且随心衰严重程度增加而升高，血浆BNP、ANP浓度（尤以BNP浓度）能较好的评价CHF患者的心功能状态。

Correlations between plasma concentration of brain natriuretic peptide, atrial natriuretic peptide and cardiac function in patients with chronic congestive heart failure

Objective: To observe the relations of the plasma brain natriuretic peptide(BNP) and atrial natriuretic peptide(ANP) concentration with chronic congestive heart failure(CHF),and investigate its clinical signicance. Method: Plasma ANP and BNP concentrations of patients with CHF(n=75) and nomal controls(n=21) were measured with immunoradiometicassay immediately after admission.LVEF was measured with echocardiography(SONO1000)at the snme time.Results: The plasma ANP and BNP concentrations of CHF group were significantly higher than those of the normal controls(P<0.01).They also increased depending on the severity of cardiac function.The plasma BNP concentration also had asignificant positive correlations with LVEF(r=-0.78,P<0.01).Conclusion: The plasma ANP,BNP concentrations of CHF increase more significantly than normal control and it may be a biochemical parameter for evaluating the left ventricular function.
