奈达铂与调强适形同步放化疗治疗中晚期食管癌疗效分析

摘    要:目的观察调强适形放疗+奈达铂同步化疗治疗中晚期食道癌的疗效及不良反应。方法将60例中晚期食道癌患者随机分为单纯调强放疗组(单纯放疗组)30例和调强放疗同步奈达铂(放化疗组)30例。2组患者均采用8MV-X线全程实施调强适形放疗,放疗剂量为95%GTV 68~70 Gy/28~30次,95%PTV 56~60 Gy/28~30次;放化疗组在放疗期间每周给予NDP 40 mg/m2静脉滴注化疗,共5.5~6星期。结果放化疗组总有效率及2年生存率高于单纯放疗组;2组白细胞减少、血小板减少、Ⅱ度放射性食道炎发生率无差异。结论同步放化疗治疗中晚期食管癌的疗效较单纯放疗明显提高,不良反应较单纯放疗无明显差异。

Abstract:Objective To investigate the clinical effects and adverse reactions of local-regional advanced esophageal carcinoma treated with concurrent IMRT and nedaplatin.Methods Sixty cases with local regional advanced esophageal were randomly divided into two groups.Thirty cases were treated with radiotherapy only,while other 30 cases were treated with 8MV-X intensive modulated radiotherapy(IMRT) and nedaplatin concurrently.All patients were radiated with IMRT,95% isodose curve of GTV 68~70 Gy/28~30 frs,95% isodose curve of PTV 56~60 Gy/28~30 frs.Nedaplatin was given weekly with the dose 40mg/m2,totally 5.5~6 weeks.Results The total effective rate of concurrent group was higher than radiotherapy group.There was no difference in adverse reactions between concurrent group and the radiotherapy alone group.Conclusion The clinical effect of concurrent chemotherapy and radiotherapy is better than that of radiotherapy alone,but there's no significant difference in adverse reactions between the two therapies.
