肠内营养支持在胰十二指肠切除术后早期应用研究

目的 :探讨早期肠内营养支持在胰十二指肠切除术后早期应用的意义。方法 :39例胰十二指肠切除患者分为观察和对照两组。观察组术后早期 (第 1天 )小肠内滴注通腑理气中药 ,待肠鸣音恢复后滴注营养液治疗 ;对照组术后早期给予静脉营养支持 ( TPN)治疗 ;两组供给热量相同。术后分别观察两组胃肠功能、营养状况、免疫功能、消化液分泌量和术后并发症。结果 :治疗组术后胃肠功能恢复明显早于对照组 ( P<0 .0 5 ) ;治疗组术后第 14天营养状况和免疫功能较术后第 1天明显改善 ( P<0 .0 5 ) ;治疗组术后消化液分泌量较对照组无明显差别 ( P>0 .0 5 ) ;并发症发生率明显低于对照组 ( P<0 .0 5 )。结论 :胰十二指肠切除术后早期实施肠内营养支持治疗 ,有助于胃肠功能早日恢复、营养状况的改善和免疫功能的提高 ,并有利于减少术后并发症的发生。

Early application study of enteral nutrition after pancreatoducodenectomy

Objective: It is to investigate the significance of early application of enteral nutrition (EN) after pancreatoduodenectomy. Methods: Thirty-nine patients of pancreatoduodenectomy were divided into treatment group and control group. Treatment group was given Chinese medicinal herbs during the early postoperative period and control group was not given herbs. When bowels movement occurred, treatment group was administered EN, and control group was administered TPN. The two groups were given isocaloric. The digestive function, nutritional status, immune function, secretive digestive quantity and postoperative complications of the two groups were observed and analyzed.Results: In treatment group, the digestive function recovered rapidly. Postoperative fourteenth day, the nutritional status and immune function of the treatment group improved markedly compared with those of postoperative first day (P<0.05). There was no significant difference in the secretive digestive quantity between the two groups. The postoperative complicationswere less than those of the control group (P<0.05). Conclusion: Early application of EN after pancreatoduodenectomy can promote the digestive function to recover and improve the nutritional status and immune function, and reduce the postoperative complications.;
