不同手术入路方式治疗第三脑室颅咽管瘤的疗效比较

目的探讨不同手术入路方式治疗第三脑室颅咽管瘤的临床治疗效果。方法选取2010年3月至2011年3月间收治的70例第三脑室颅咽管瘤患者,按照随机数字法分为A组和B组,每组35例。A组患者给予经皮质-侧脑室入路手术,B组患者给予经胼胝体-透明隔-穹窿间入路手术,观察两组患者的临床治疗效果。结果 A组患者肿瘤全切除率为77.1%,B组患者肿瘤全切除率为74.3%,两组差异无统计学意义(P>0.05)。A组患者手术时间和术中出血量分别为(127.3±23.5)min和(24.2±7.4)ml,B组患者手术时间和术中出血量分别为(121.6±24.1)min和(23.3±7.2)ml,两组差异无统计学意义(P>0.05)。A组患者术后并发症发生率为20.0%,B组为17.1%,两组差异无统计学意义(P>0.05)。结论对第三脑室颅咽管瘤患者采取经皮质-侧脑室入路手术和经胼胝体-透明隔-穹窿间入路手术治疗均具有较好的应用效果,临床可以选取这两种入路方式进行手术治疗,提高患者的临床治疗效果。

Effect comparison of two kinds of operation approach for the third ventricle craniopharyngiomas

Objective To investigate the clinical therapeutic effect of different operation approach in the treatment of the third ventricular craniopharyngeal duct tumor. Methods Selected in our hospital from 2010 March to 2011 March included 70 cases of third ventricle craniopharyngiomas were randomly di- vided into A group and B group, A group of 35 patients treated by cortex -lateral ventricle approach operation, B group of 35 patients treated with corpus callous -septum pellucid -fornix in operation, the efficacy of the two groups. Results Through comparative analysis of clinical therapeutic effect of two groups, patients in group A total tumor resection rate was 77.1% , B group of patients with total tumor resection rate was 74. 3% , no significant differences between two groups of data （P 〉0. 05） , no statistical significance. The amount of bleeding in patients of group A operation time and intraoperative were （ 127. 3 ± 23.5 ） min, （24.2 ±7.4） ml, amount of bleeding in B group operation time and intraoperative were （ 121.6 ±24. 1 ） rain, （23.3 ±7.2） ml, A and B groups of patients with operation time and bleeding no significant difference of the amount （ P 〉 0. 05 ） , no statistical significance. The complication rate was 20% after operation in a group; the complication rate was 17. 1% after operation in B group, no significant differences between two groups of data （ P 〉 0. 05 ）, no statistical significance. Conclusion In clinical practice, third ventricle eraniopharyngiomas were treated by cortex -lateral ventricle approach operation and transcallosal -septum pellucid -interfornieeal approach operation treatment has a better effect, both in the treatment of tumor resec- tion and operation time and the intraoperative complications and postoperative bleeding similar clinical, can choose the two approach of operation treatment, improve the clinical therapeutic effect.
