三维适形放射治疗联合化疗同步治疗晚期非小细胞肺癌的临床分析

目的探讨三维适形放射治疗联合化疗同步治疗晚期非小细胞肺癌的临床疗效。方法对86例晚期非小细胞肺癌患者临床资料进行回顾性分析,其中单纯化疗组40例,三维适形放射治疗联合化疗组46例。比较两组患者近期疗效、1、2、3年生存率和不良反应发生率。结果放射治疗联合化疗组和单纯化疗组治疗总有效率分别为84.8%和55.0%,差异有统计学意义(P<0.05)。放射治疗联合化疗组1、2、3年生存率分别为54.3%、30.4%和10.9%,单纯化疗组分别为32.5%、12.5%和0(P<0.05)。放射治疗联合化疗组放射性食管炎和放射性肺炎发生率分别为23.9%和2.2%,两组患者消化道反应及白细胞下降发生率差异无统计学意义(P>0.05)。结论三维适形放射治疗联合化疗可显著提高晚期非小细胞肺癌生存率,不良反应小,患者可耐受。

Effect of 3D-CRT combined with chemotherapy for advanced non-small cell lung cancer

Objective To explore the effect of three dimension-conformed radiotherapy （3D-CRT） combined with chemotherapy for advanced non-small cell lung cancer. Methods The dates of 86 eases with advanced non-small cell lung cancer were retrospectively analyzed, including 40 patients undergoing chemotherapy alone and 46 patients performed 3D-CRT combined with chemotherapy. Short term effect, 1, 2 and 3 year survival rates and adverse rates were compared between two groups. Results The effective rates were 84. 8% and 55.0% in 3D-CRT and chemotherapy group and chemotherapy group respectively with significant difference （P〈0. 05） ; 1,2 and 3 year survival rates were 54. 3%, 30. 4% and 10. 9% in 3D-CRT and chemotherapy group, which were much higher than those in chemotherapy group （ 32. 5% , 12. 5% and 0, P 〈 0. 05 ）. The occurrence of radiation esophagitis and radiation pneumonitis in 3D-CRT and chemotherapy group were 23.9% and 2. 2%. There were no significant difference of gastrointestinal reaction and leukocyte decrease between two groups （ P 〉 0.05 ）. Conclusion 3 D-CRT combined with chemothera- py for advanced non-small cell lung cancer greatly improve the survival rate with little adverse reaction and well tolerated.
