782例甲状腺乳头状癌患者的临床特点分析及临床治疗体会

目的探讨甲状腺乳头状癌的临床特点及诊疗手段。方法回顾性分析了我院近年来收治的782例甲状腺乳头状癌患者的临床资料,对其癌肿特点、淋巴结转移及手术方式进行了总结,并对术后并发症及恢复情况进行了随访。结果所有患者均为甲状腺乳头状癌,大部分患者为单侧发病,TMN分级为Ⅰ期,直径超过1cm及癌肿突破甲状腺胞膜及癌肿侵及颈前肌肉时病例淋巴结转移率明显增高,术后34例患者出现了并发症,经相关治疗后恢复,41例患者出现局部复发,无死亡病例。结论应根据患者实际情况及术中病检结果选择手术及淋巴结清扫方式,同时应注意手术并发症的发生。

Analysis and Summarizing for the Clinical Character and Treatment Experience of 782 Thyroid Papillary Carcinoma Cases

Objective Exploring the clinical features of papillary thyroid carcinoma and treatment means. Method Clinical data of 782 papillary thyroid cancer cases admitted to our hospital in recent years were retrospectively analyzed. The character of carcinoma, lymph node metastasis and surgery method were summarized. The post-operative complications and prognosis were followed up. Results All the selected patients were diagnosed to thyroid papillary carcinoma by pathological ways during surgery. The bumps in most of the patients were unilateral. The rate of lymph node metastasis of the patients with bumps diameter over lcm, breakthrough thyroid membrane and invading the anterior cervical muscle were significantly increased. The complications were found in 34 cases and they were fully recovered after treatment. 41 cases were recurred after surgery. No patient died in this research. Conclusion The surgery ways and lymph node metastasis ways should be selected by the practical condition of patients. And doctors should pay attention to the complications.
