经皮导管肾脏交感神经射频消融术治疗顽固性高血压的研究进展

摘    要:药物治疗很难使顽固性高血压患者的血压达标,其靶器官损害发生率明显升高,因而积极探寻新型降压疗法具有重要意义。肾脏交感神经活动增强是高血压发生、发展的重要机制。经皮导管肾脏交感神经射频消融术(RSD)治疗顽固性高血压,开创了原发性高血压非药物治疗的新途径。基础和临床研究已初步证实RSD治疗顽固性高血压是安全有效的,其还可能用于治疗其他伴有交感神经过度激活的疾病,如左心室肥厚、慢性心力衰竭、睡眠呼吸暂停综合征、胰岛素抵抗、慢性肾病等。

Abstract:It is difficult for drug therapy to keep the blood pressure of patients with refractory hypertension within the standard range, which would cause significant rising of the incidence of target organ damage, therefore,it has great significance in exploring new antihypertensive treatment method. Hypertension occurs and develops because of the activation of renal sympathetic nerve. Catheter-based renal sympathetic denervation(CRSD/RSD) opens up a new way of non-drug treatment for primary hypertension,its safety and effectiveness has been demonstrated by basic and clinical research. Moreover, RSD can also be applied to treat other diseases with the activation of sympathetic nerve, such as left ventricular hypertrophy, chronic heart failure disease, sleep apnea syndrome, insulin resistance and chronic kidney disease.
