ICU患者发生高钠血症相关危险因素分析

目的研究ICU患者高钠血症的发生情况及其发生的相关危险因素。方法对2011年1月至2013年5月在梧州市人民医院ICU住院的543例患者的高钠血症发生情况进行研究,采用单因素分析和多因素非条件Logistic回归分析ICU患者发生高钠血症的相关危险因素。结果 87例患者并发高钠血症,高钠血症的发生率为16.02%,单因素和多因素分析结果显示:患有糖尿病(OR=6.564,95%CI 0.456⁶.445),有颅脑损伤(OR=5.535,95%CI 0.0366.445),有颅脑损伤(OR=5.535,95%CI 0.036⁴.634),机械通气治疗(OR=3.646,95%CI 0.5634.634),机械通气治疗(OR=3.646,95%CI 0.563⁴.645),饮水<500 mL/d(OR=3.556,95%CI 0.4564.645),饮水<500 mL/d(OR=3.556,95%CI 0.456⁶.546),随机血糖(OR=5.645,95%CI 0.6746.546),随机血糖(OR=5.645,95%CI 0.674⁶.335)是ICU患者发生高钠血症的相关危险因素。结论 ICU患者发生高钠血症的比例较高,治疗的过程应当重视上述危险因素,制订合理的预防措施。

Analysis on the Risk Factors of Hypernatremia among Patients in Intensive Care Unit

Objective To investigate the incidence and risk factors of hypernatremia among patients in intensive care unit.Methods A total of 543 cases of patients with hypematremia admitted in ICU,Wuzhou People＇s Hospital from Jan.2011 to May 2013 were collected.Risk factors of hypernatremia were analyzed by univariate and multivariate non-conditional Logistic regression analysis.Results A total of 87 cases were complicated by hypematremia,that the incidence rate of hypernatremia was 16.02％.Results of univariate and multivariate analysis showed：diabetes （OR =6.564 ; 95％ CI：0.456-6.445）,brain injury （OR =5.535 ;95 ％ CI：0.036-4.634）,mechanical ventilation （OR =3.646 ; 95 ％ CI：0.563-4.645）,water intake ＜ 500 mL/d （OR =3.556 ; 95 ％ CI：0.456-6.546）,random blood glucose （OR =5.645 ; 95 ％ CI：0.674-6.335） were the risk factors of hypernatremia among patients in ICU.Conclusion Incidence rate of hypernatremia of patients in ICU is high,and related risk factors of hypernatremia should be paid attention to in the treatment,in order to carry out reasonable preventive measures.
