放射状视神经切开术治疗视网膜中央静脉阻塞的可行性及临床价值

目的探讨放射状视神经切开术治疗视网膜中央静脉阻塞(CRVO)的可行性及临床价值。方法选择2011年3月至2013年2月邯郸市眼科医院收治的34例(43只眼)CRVO患者作为研究对象,患者均采用放射状视神经切开术治疗,分析术后1周、1个月、3个月、6个月患者的视力、眼底、荧光血管造影的检查结果。结果 34例患者的手术均成功,术后1个月、3个月、6个月患者的视力较手术前显著改善(P<0.05)。患者均成功随访6个月,术后1个月视网膜或黄斑水肿减轻或消退;术后3个月眼底出血吸收、视盘和黄斑水肿消退;术后6个月眼底荧光血管造影显示视网膜静脉通畅。术后无严重并发症发生。结论放射状视神经切开术治疗CRVO能够显著提高患者的视力、改善患者的临床症状、体征,值得临床推广应用。

Feasibility and Clinical Value of Radial Optic Neurotomy in Treatment of Central Retinal Vein Occlusion

Objective To explore the feasibility and value of radial optic neurotomy in the treatment of central retinal vein occlusion （CRVO）.Methods Total of 34 cases （43 eyes） of CRVO from Mar.2011 to Feb.2013 in Handan Ophthalmologic Hospital were analyzed：all of them were treated with radial optic neurotomy,and the results of visual acuity,eyeground and fluorescein angiography at 1 week,1 month,3 months,6 months after surgery were analyzed.Results All of the 34 operations were successful,but the visual acuity at 1 month,3 months,6 months after surgery were significantly higher than before surgery（P ＜ 0.05）.All patientswere successfully followed up for 6 months,retina or macular edema were relieved or dissipated after 1 month,and after 3 months,retinal hemorrhage were assimilated,optic disc and macular edema were dissipated,after 6 months,fluorescein angiography revealed retinal vein patency.There was no serious complication after surgery.Conclusion Radial optic neurotomy in treatment of CRVO can improve the visual acuity,improveclinical symptoms and signs,thus is worthy of clinical application.
