肩关节置换术治疗肱骨近端骨折的研究进展

肱骨近端骨折多见于中老年患者,多采用肩关节置换术治疗。如果骨折造成骨性标志破坏、术中假体放置高度及后旋角度不确定、大小结节未达到解剖复位等可影响肩关节置换术后的疗效及预后。因此,对于肱骨近端骨折的治疗,需熟悉肩关节的解剖关系、力学机制、肩关节特殊的重建技术,并制订周密的手术方案才可以降低并发症的发生率。现就近几年来肩关节置换术治疗肱骨近端骨折的情况予以综述。

Research Progress of Shoulder Joint Replacement in Treating Proximal Humeral Fractures

Proximal humeral fractures most frequently occur to middle and old-aged patients, and the major treatment is shoulder joint replacement. However, therapeutic effects and prognosis after shoulder joint replacement can be influenced by bony landmarks damage, the uncertainty of placement height and back rotation angle of prosthesis, greater tubercle and lesser tubercle not meeting the requirements of anatomical reduction and so on. Therefore,in order to treat the proximal humeral fractures and lower the occurrence rate of postoperative complications, doctors need to be familiar with the anatomic relationships between shoulder joints,the mechariics mechanism and the special reconstruction technology of the shoulder joints and make meticulous operation plans. Here is to make a review about the situations of treating the proximal humeral fractures through shoulder joint replacement in recent years.
