骨髓增生异常综合征15例临床回顾性分析

目的探讨骨髓增生异常综合征(MDS)的临床诊断与治疗方法。方法回顾性分析2010年2月至2011年8月哈尔滨医科大学附属第一医院血液科收治的15例MDS患者的临床资料,并结合文献对MDS的发病特点及治疗进行讨论。结果本组15例转化为急性白血病5例,其中急性淋巴细胞性白血病2例,急性髓细胞性白血病2例。总的急变率为33.3%。采用化疗及抗细菌真菌治疗,病情稳定后出院。结论 MDS中国发病年龄较西方提前10岁左右,诊断难点在于鉴别诊断。中、高危MDS患者建议使用地西他滨联合化疗方案。

A Retrospective Analysis of the Clinical Features of 15 Cases of Myelodysplastic Syndrome

Objective To improve the clinical diagnosis and treatment of myelodysplastic syndrome （MDS）. Methods A retrospective analysis of clinical data of 15 cases of MDS from Department of Hematology of the First Affiliated Hospital of Harbin Medical University, from Feb. 2010 to Aug. 2011 was done, and the relevant literature was referred to discuss the onset characteristics and treatment of the disease. Results This group included 15 patients,5 cases had transformed to acute leukemia, including acute lymphohlastic leukemia in 2 cases, acute mydoid leukemia in 2 cases. The total transformation rate was 33.3%. Patients received chemotherapy and anti-bacterial-fungal treatment, and were discharged in stable condition. Conclusion The onset age of MDS in China was about 10 years earlier than the west, the differential diagnosis is the difficult diagnosis point. It＇s suggested that decitabine combined chemotherapy be used for middle and high risk MDS patients.
