全胸腔镜肺叶切除手术治疗复杂原发性肺癌的疗效观察

目的对复杂原发性肺癌采用全胸腔镜肺叶切除手术治疗的临床效果进行观察和分析。方法选择60例复杂原发性肺癌的患者按照手术方法的不同分为对照组和治疗组,对照组患者采用传统开胸肺叶切除手术治疗,治疗组给予全胸腔镜肺叶切除手术治疗,比较两组患者的术中失血量、清扫的淋巴结数量、切口疼痛程度、引流时间、手术时间、术后胸腔引流量、术后并发症、住院时间和术后生存率等。结果治疗组患者清扫的淋巴结数量和术后生存率与对照组比较无明显差异（P〉0.05）;治疗组患者的术中失血量、切口疼痛程度、引流时间、手术时间、术后胸腔引流量、术后并发症、住院时间均小于对照组,差异有统计学意义（P〈0.05）。结论复杂原发性肺癌采用全胸腔镜肺叶切除手术治疗有显著的效果,值得临床推广。

Observation on the efficacy of completely thoracoscopic lobectomy for treatment of complex primary lung cancer

Objective To observe and analyze the clinical effect of completely thoracoscopic lobectomy for treatment of complex primary lung cancer. Methods A total of 60 patients with complex primary lung cancer were selected and assigned to the control group and the treatment group according to surgical procedures. The control group was treated by lobectomy via conventional thoracotomy and the treatment group was treated by completely thoracoscopic lobectomy.The intraoperative blood loss, the number of dissected lymph nodes, degree of incision pain, duration of drainage, duration of surgery, postoperative chest drainage volume, postoperative complications, hospital stay and postoperative survival rate were compared between the two groups. Results The treatment group and the control group had no statistical difference with regard to the number of dissected lymph nodes or postoperative survival rate（P〉0.05）; the treatment group had less intraoperative blood loss, slighter incision pain, shorter duration of drainage, shorter duration of surgery,less postoperative chest drainage volume, fewer postoperative complications and shorter hospital stay than the control group, with statistical significance（P〈0.05）. Conclusion Completely thoracoscopic lobectomy demonstrates excellent effect for treatment of complex primary lung cancer, which is worthy of clinical promotion.
