应激性高血糖对老年急性冠脉综合征的预后影响

目的探究应激性高血糖对老年急性冠脉综合征的预后影响。方法将我院诊治的120例老年急性冠脉综合征患者,按照入院时血糖检测结果,分为A组（〈7.8 mmol/L）60例、B组（7.8 mmol/L～11.1 mmol/L）28例、C组（〉11.1 mmol/L）32例,回顾性分析三组患者的临床资料,对比三组患者的预后状况。结果随着血糖水平的上升,心力衰竭、心源性休克、恶性心律失常、心功能≥Ⅲ级等发生率及血清心肌肌钙蛋白I（c Tn I）、N末端脑钠肽前体（NT-pro BNP）,均呈逐渐升高趋势,其中,C组心力衰竭、心源性休克、恶性心律失常、心功能≥Ⅲ级等发生率及c Tn I、NT-pro BNP等指标水平均显著高于A组,差异均有统计学意义（P〈0.05）;C组住院期间死亡率为21.88%,显著高于A组1.67%、B组3.57%,差异均有统计学意义（P〈0.05）。结论应激性高血糖可提示老年急性冠脉综合征患者的预后状况不良,当血糖水平〉11.1 mmol/L时,患者的死亡风险增加。

Effect of the prognosis of stress hyperglycemia on the patients with acute coronary syndrome

Objective To explore the effect of the prognosis of stress hyperglycemia on the patients with acute coronary syndrome. Methods A total of 120 cases of elderly patients with acute coronary syndrome in our hospital for treatment,according to admission blood glucose detection results,they were divided into 60 cases of group A（7.8mmol/L）,28 cases of group B（7.8 mmol/L-11.1 mmol/L）,32 cases of Group C（11.1 mmol/L）, retrospectively analyzed the clinical data of three groups of patients,the prognosis of the three groups were compared. Results With the rising of blood sugar levels,heart failure,cardiogenic shock,malignant arrhythmia, occurrence rate of heart function ≥Class Ⅲ and index levels of serum cardiac troponin I（c Tn I）N,N-terminal pro brain natriuretic peptide（NT-pro BNP） were all significantly higher than those of group A, the difference had statistical significance（P0.05）;The mortality rate of group C was 21.88%,significantly higher than that of 1.67% of group A,3.57% of group B, the difference had statistical sig nificance（P0.05）. Conclusion Stress hyperglycemia can predict bad condition of the prognosis of elderly patients with acute coronary syndrome, when the blood glucose level is 11.1mmol/L,risk of death of patients increases.
