肺炎支原体肺炎患儿免疫功能的临床分析

摘    要:目的研究不同程度的肺炎支原体肺炎患儿T细胞亚群、免疫球蛋白水平变化,探讨其免疫功能以为临床治疗提供依据。方法用流式细胞仪检测肺炎支原体肺炎患儿急性期(重症组、普通组)和正常对照组儿童外周血T细胞亚群,用免疫比浊法检测血清Ig A、Ig M、Ig G水平。结果肺炎支原体肺炎重症组T细胞亚群CD3+、CD4+、CD4+/CD8+低于普通组和对照组(P〈0.01),普通组T细胞亚群CD3+、CD4+、CD4+/CD8+低于对照组(P〈0.01);CD8+重症组高于普通组(P〈0.01),而在普通组和对照组差异无统计学意义(P〉0.05)。普通组和重症组肺炎血清Ig A、Ig M、Ig G水平均高于正常对照组(P〈0.01),但Ig G重症组低于普通组,差异有统计学意义(P〈0.01)。结论肺炎支原体肺炎患儿存在细胞免疫和体液免疫功能失调,检测T细胞亚群、免疫球蛋白水平,有利于判断肺炎支原体肺炎患儿病情,为临床应用免疫调节剂提供理论依据。

Clinical significance of immune function of the children with mycoplasma pneumoniae pneumonia

Abstract:Objective To study the levels of T-Lymphocyte subsets and Immunoglobulin of children with varying de- grees of mycoplasma pneomoniae pneumonia, discussed the immunity function and provide a basis for clinical treat- ment. Methods Peripheral blood samples were collected from the children with mycoplasma pneumoniae pneumonia and healthy children. Then T-Lymphocyte subsets were detected by flow cytometry, and the IgA, IgM, IgM and IgG were tested by the immunoturbidimetric assay. Results The percentages of T-Lymphocyte subsets CD3~, CD4+, CD4~/ CD8~ in severe mycoplasma pneumoniae pneumonia group were lower than those in control and mild group (P〈0.01), the percentages of T-Lymphocyte subsets CD3+, CD4~, CD4~/CD8~ in mild group were lower than those in control group (P〈0.01), CD8~ in severe mycoplasma pneumoniae pneumonia group were higher than mild group (P〈0.01), but the percentages of CD8+ were not significantly different between control and mild group (P〉0.05). The concentrations of IgA, IgM and IgG in sever group and mild group were higher than those in control group (P〈0.01). But the percentages of IgG in sever group were lower than those in control, the difference were statistically significant (P〈0.01). Conclusion Chil- dren with MPP have cellular immune and humoral immune disorders. Through the detection of T-lymphocyte subsets and immunoglobulin, it will be helpful to judge the patients's condition, which provides a theoretical basis for the clin- ical application of immune regulators.
