云南省2006～2009年孕产妇产科出血死亡原因及相关因素分析

目的:探讨孕产妇产科出血死亡原因及影响因素,制定切实可行的干预措施。方法:对2006~2009年云南省孕产妇死亡监测及评审资料中的469例产科出血资料进行回顾性分析,采用PMES 3.0软件对孕产妇的死因构成、分娩地点、死亡地点、孕次、产次等进行单因素分析及χ2检验,使用SPSS 15软件对产科出血进行Logistic回归分析。结果:4年来产科出血死亡率呈逐年下降趋势,但仍居全省孕产妇死亡的首位原因。469例产科出血死亡孕产妇中,产后出血占78.90%,子宫收缩乏力及胎盘滞留是产后出血死亡的主要原因,分别占34.10%及30.70%。影响孕产妇死亡的主要因素:交通、经济、文化等的滞后导致孕妇不能住院分娩;孕期保健服务质量差及县、乡产科人员业务技术水平偏低。结论:加强孕期保健服务、提高县乡两级产科人员业务技术水平是降低孕产妇产科出血死亡率的关键;切实解决交通不便的边远少数民族地区孕产妇分娩安全,是降低孕产妇死亡率不容忽视的问题。
