颈动脉内中膜厚度及横截面积与冠心病关系的分析

摘    要:目的通过颈动脉内-中膜厚度(IMT)及横截面积(IMCSA)的测定,分析颈动脉粥样硬化与冠心病的关系.方法对128例接受动脉造影的患者行颈动脉超声检查并通过应用自主研发的计算机智能跟踪血管影像分析系统测量颈动脉IMT及IMCSA,比较有无冠脉病变及不同程度冠脉病变与颈动脉IMT、IMCSA的关系.结果非冠心病者45例,冠心病单支病变组39例,多支病变组44例.冠心病患者的颈动脉两冠心病组颈动脉IMT均较对照组增厚(P〈0.05),颈动脉IMCSA较对照组明显增大(P〈0.01),并且随着冠状动脉病变累及支数的增加,IMT明显增加,IMCSA明显增大,多支病变组与单支病变组比较差异有统计学意义(P〈0.01).结论颈动脉IMT及IMCSA可作为间接反映冠状动脉病变程度的指标;计算机智能跟踪血管影像分析系统测量准确,使用简便,值得推广应用.

Abstract:Objective To analyze the relationship between atherosclerosis and coronary heart disease by determining the intima-medial thickness(IMT)and cross-sectional area(IMCSA)of carotid artery.Methods 128 patients who received vein angiography were given carotid artery ultrasound examination and self-development through the application of auto-tracing vessel image analysis system to track vessel carotid artery IMT and IMCSA,with or without coronary artery disease and different degrees of coronary disease and carotid artery IMT,IMCSA relationship.Results There were 45 patients without coronary heart disease,39 patients with coronary heart disease(single vessel disease),and 44 patients with coronary heart disease(multi-vessel disease).Carotid IMT of patients with coronary heart disease was thicker than patients without coronary heart disease in control group(P0.05).Carotid artery IMCSA were markedly increased in patients with coronary heart disease(P0.01),and increased with the increase in the number of involved branches.IMT significantly increased,IMCSA significantly increased,there was significant difference between multi-vessel disease group and single-vessel disease group(P0.01).Conclusions Carotid IMT and IMCSA can be used as indirect indicators reflecting the degree of coronary artery disease.Computer auto-tracing vessel image analysis system can accurately track vessel,and is easy to use,should be widely applied.
