下肢深静脉血栓导管溶栓效果的相关因素分析

目的探讨下肢深静脉血栓（DVT）导管接触性溶栓效果的相关因素。方法回顾性分析2006年1月-2008年9月175例接受导管接触性溶栓治疗下肢DVT患者的临床资料,将本组资料所有患者根据导管接触性溶栓效果分成治愈和未愈两组,进行单因素分析和多因素logistic回归分析。结果单因素分析显示,病因、年龄、病程、发病诱因、合并基础疾病、尿激酶剂量不同组别间治愈率有统计学差异（P〈0.05）。多因素logistic回归分析结果显示：病因、尿激酶、病程不同组别间治愈率有统计学意义（P〈0.05）。结论有无明确病因、尿激酶剂量、病程长短是接触性溶栓效果的独立相关因素,推荐有明确病因、病程〈1月的DVT患者采用大剂量尿激酶导管接触性溶栓。

Related factors to influence catheter-directed thrombolysis in lower extremity deep venous thrombosis

Objective To assess the related factors to influence catheter-directed thrombolysis in lower extremity deep venous thrombosis（DVT）.Methods From January 2006 to September 2008 175 cases with lower limb DVT were treated,and clinical data of patients were retrospectively reviewed.According to the contact thrombolytic effect patients were divided into two groups.Data analysis was done with univariate analysis and multivariate logistic regression.Results The total effective rate of thrombolysis was 77.7%.Univariate analysis showed that age,course of disease,cause,consolidated basis disease,dose urokinase were related to the effective rate.Multivariate logistic regression analysis showed that cause,dose urokinase,course of disease were related to the cure rate.Conclusion DVT treatment is affected by many factors,whether cause and dose urokinase as well as course of disease are major dependent factor for catheter-directed thrombolysis.Clear etiology,course 1 month of DVT treated with high dose urokinase thrombolysis catheter contact is recommended.
