针刀治疗神经根型颈椎病的临床研究

摘    要:目的通过对针刀治疗神经根型颈椎病疗效的观察,为本病的临床治疗提供一种疗程短、见效快、疗效确切的治疗方法。方法将64例2015年5月-2018年12月就诊于泉州市中医院针灸科门诊、住院部、符合纳入标准并同意接受本研究治疗方案的神经根型颈椎病患者,采取随机对照试验的临床研究方法,按1:1比例随机分为对照组和治疗组,每组32例,对照组采用传统针刺方法治疗,治疗组采用针刀疗法治疗,以3周为治疗时间,在治疗前、首次治疗后、足疗程治疗后、治疗结束后1个月进行NPQ量表、McGill疼痛评分量表的测评和评分,并进行统计分析。结果 2组治疗前后NPQ评分和McGill评分的组内评分,治疗组治疗前、首次治疗后、足疗程治疗后、治疗结束1个月后评分分别与前次比较有统计学意义(P <0.05);对照组首次治疗后较治疗前比较无统计学意义(P> 0.05),足疗程治疗后、治疗结束后1月评分分别与前次比较有统计学意义(P <0.05)。表明传统针刺法和针刀疗法均对神经根型颈椎病的治疗有效。2组组间的评分比较有统计学意义(P <0.05),表明治疗组采用的针刀疗法优于对照组的传统针刺方法,两组NPQ评分和McGill评分值在各观察时点均有下降。在首次治疗后、结束后1月(P <0.05),表明两组效果具有差异性,治疗组具有较好的近期及远期效应。结论(1)针刀疗法、传统针刺对于神经根型颈椎病的治疗均获得良好的临床疗效,均能缓解和改善患者的临床症状。(2)针刀疗法与传统针刺相比,有较好的近期及远期效应。

Abstract:Objective To provide a short course, quick effect and definite curative effect for the clinical treatment of cervical spondylotic radiculopathy by observing the curative effect of acupotomy in the treatment of cervical spondylotic radiculopathy. Methods The subjects of this study were 64 patients with cervical spondylotic radiculopathy who met the inclusion criteria and agreed to accept the treatment plan of this study. All the patients came from the outpatient and inpatient in the departments of the acupuncture and moxibustion in quanzhou hospital of traditional Chinese medicine. The 64 patients were randomized into control group and treatment group according to 1:1 ratio, 32 in each group. The control group was intervened by traditional acupuncture and the treatment group was treated with needle-knife therapy. The duration of treatment was 3 weeks. NPQ scale and McGill pain scale were assessed and scored before treatment, after the first treatment, after the treatment course and one month after the treatment, and statistical analysis was made. Results The NPQ score and MCGill score before and after treatment in the two groups had statistical significance(P < 0.05) compared with the previous score before treatment, after the first treatment, after the treatment course and one month after the end of treatment in the treatment group, while there was no statistical significance in the control group after the first treatment(P > 0.05). The scores after the full course of treatment and one month after the treatment were statistically significant compared with the previous one(P < 0.05). It showed that both traditional acupuncture and needle-knife therapy were effective in the treatment of cervical spondylotic radiculopathy. The scores between the two groups were statistically significant(P < 0.05), indicating that the needle-knife therapy in the treatment group was superior to the traditional acupuncture method in the control group. The NPQ score and McGill score in the two groups decreased at different time points. After the first treatment and one month after the end of the treatment(P < 0.05), the results showed that there were differences between the two groups, and the treatment group had better short-term and long-term effects. Conclusion(1) Needle-knife therapy and traditional acupuncture have good Clinical Effect in the treatment of cervical spondylotic radiculopathy, which can alleviate and improve the clinical symptoms of patients with cervical spondylotic radiculopa.(2)Compared with traditional acupuncture and moxibustion, needle-knife therapy has better short-term and long-term effects.
