血清维生素D水平与女童ICPP的相关性研究

摘    要:目的了解女童特发性中枢性性早熟与维生素D水平的相关性及临床意义。方法对医院2016年8月-2019年8月在医院儿科门诊诊断ICPP的女童180例,检测其空腹总25羟维生素D(Vit D),25羟维生素D2(Vit D2)及25羟维生素D3(Vit D3),同时测定促黄体生成素(LH)、卵泡刺激素(FSH)、雌二醇(E2)、胰岛素样生长因子(IGF-1)、骨龄及体块指数(BMI),160例同龄女孩作为对照组。结果观察组与对照组的VitD、VitD2及VitD3水平有显著差异。VitD观察组(62.17±11.99)nmol/L,对照组(82.71±26.69)nmol/L,P=0.003];Vit D2观察组(3.16±0.62)nmol/L),对照组(4.17±1.36)nmol/L,P=0.002];Vit D3观察组(59.05±11.34)nmol/L,对照组(77.87±25.03)nmol/L,P=0.030];其中ICPP组有83例(46.1%)维生素D缺乏,68例(37.8%)维生素D不足,29例(16.1%)维生素D正常。ICPP组维生素D缺乏组存在明显的差异,有统计学意义,且维生素D、D2、D3浓度与峰值LH/峰值FSH、骨龄/生活年龄、IGF-1的差异呈显著负相关。结论维生素D、D2、D3的水平与女童ICPP密切相关。

Study on the Relationship Between Vitamin D Level and the ICPP of Girls

Abstract:Objective To investigate the correlation and clinical significance between Idiopathic central precocious puberty and vitamin D level in girls. Methods From August 2016 to August 2019, 180 girls with ICPP were diagnosed in paediatrics clinic of our hospital. Fasting total 25 hydroxyvitamin D(Vit D), 25 hydroxyvitamin D2(Vit D2) and 25 hydroxyvitamin D3(Vit D3) were measured,(LH), follicular stimulating hormone(FSH), estrogen(E2), insulin-like growth factor(IGF-1), bone age and body mass index(BMI), were measured in 160 girls of the same age. As the control group. Results There were significant differences in the levels of Vit D, Vit D2 and Vit D3 between the observation group and the control group. Vit D (62.17±11.99) mmol/L in the observation group,(82.71±26.69) nmol/L, P =0.003];Vit D2 in the control group(3.16±0.62)nmol/L in the observation group and(4.17±1.36)nmo l/L in the control group, P ≤ 0.002], Vit D3 observation group(59.05±11.34) nmol/L,(77.87±25.03) mmol/L, P ≤ 0.030], 83 cases(46.1%) vitamin D deficiency, 68 cases(37.8%) vitamin D deficiency and 29 cases(16.1%) vitamin D normal in ICPP group. There was significant difference between Vitamin D, D2 and D3 concentrations were negatively correlated with peak LH/peak FSH, bone age/living age and IGF-1. Conclusion The levels of vitamin D, D2 and D3 are closely related to ICPP in girls.
