自拟息风通瘀开窍方对急性腔隙性脑梗死患者颈动脉平均血流量、MMSE评分及肢体活动能力的影响

目的观察自拟熄风通瘀开窍方对急性腔隙性脑梗死患者颈动脉平均血流量、简易智力状态检测量表(MMSE)评分及肢体活动能力的影响并探讨其作用机制。方法 300例患者按照入院先后顺序随机划分为两组,对照组与治疗组各150例。对照组均给予常规神经内科治疗,治疗组在对照组基础上应用自拟息风通瘀开窍方治疗。对比两组临床疗效、中医证候积分、神经功能缺损情况、脑血流动力学指标、认知功能以及肢体活动能力。结果治疗组治疗后总有效率为94.00%,高于对照组的78.00%(P <0.05)。两组中医证候积分、美国国立卫生研究院卒中量表(NIHSS)评分与治疗前比较均降低(均P <0.05),且治疗组均低于对照组(均P <0.05);治疗组梗死侧脑血流动力学各项指标与治疗前比较均有改善(均P <0.05),且治疗组改善情况优于对照组(均P <0.05);治疗组MMSE及简式Fugl-Meyer运动功能评分法(FMA)与治疗前比较均增高(均P <0.05),且治疗组均高于对照组(均P <0.05)。结论对急性腔隙性脑梗死患者实施自拟息风通瘀开窍方治疗临床疗效显著,可明显降低神经功能缺损程度,有效改善脑血流动力学指标、认知功能以及肢体活动能力。

The Effects of Self-made Xifeng Tongyu Kaiqiao Decoction on Average Carotid Artery Blood Flow,MMSE Score and Limb Activity in Patients with Lacunar Cerebral Infarction

Objective:To explore the effects of self-made Xifeng Tongyu Kaiqiao Decoction on average carotid artery blood flow,MMSE score and limb activity in patients with acute lacunar cerebral infarction. Methods:300 patients were divided into the control group and the treatment group,according to the order of admission,150 cases in each. The control group was given routine neurological treatment,while the treatment group was treated with self-made Xifeng Tongyu Kaiqiao Decoction on that basis. The clinical efficacy,TCM syndrome score,neurological deficit,cerebral hemodynamic index,cognitive function,and limb activity were compared between the two groups.Results:The total effective rate of the treatment group was 94.00%,higher than 78.00% of the control group(P <0.05);the score of TCM syndrome and NIHSS in both groups were lower than those before treatment(P < 0.05),and the treatment group were lower than the control group(P < 0.05);the indexes of cerebral hemodynamics in the treatment group were improved compared with those before treatment(P < 0.05),and the improvement in the treatment group was better than that in the control group(P < 0.05);MMSE and FMA of the treatment group were higher than those of the control group before treatment(P < 0.05),and the treatment group was higher than the control group(P < 0.05). Conclusion:Self-made Xifeng Tongyu Kaiqiao Decoction is effective on acute lacunar cerebral infarction,and can significantly reduce the degree of neurological deficit and improve cerebral hemodynamics,cognitive function and limb activity.
