重症急性胰腺炎的护理研究

目的探讨重症急性胰腺炎(SAP)的护理效果和经验。方法对2006年1月～2009年1月间在我院住院治疗的SAP患者50例进行前瞻性分析,总结护理效果和经验。结果本组均行手术治疗,22例行胰腺及周围坏死组织清除,27例行腹腔灌洗引流,同时行空肠造瘘、胃造瘘、胆囊造瘘、胆管引流、盆腔引流。本组痊愈36例,4例病人术后7天恢复欠佳转上级医院继续治疗,死亡2例,死亡原因为患者高龄且术后并发多脏器功能衰竭。本组早期休克8例,电解质紊乱23例,糖尿病9例,胰腺假性囊肿3例,多脏器功能衰竭2例。结论全面系统的护理有利于患者的康复,对于改善预后降低病死率至关重要。

Nursing Research on Patients with Severe Acute Pancreatitis

Objective To investigate the nursing effects and experiences of severe acute pancreatitis.Method 50 cases with severe acute pancreatitis in our hospital from January in 2006 to January in 2009 were analyzed prospectively.Results All of the patients received the operation treatment,in which 22 cases received the clearance operation of heart sweetbread and necrotic tissue in environment,and 27 cases received the peritoneal lavage and draining,and with jejunum stoma,stomach stoma,cholecyst stoma,bile duct draining and pelvic drainage.36 cases cured,4 cases were with hospital referral because of bad recovery for further treatment,in which 2 cases died,and the dead reasons were old ages with MOF.There were 8 cases with early shock,23 cases with electrolyte disturbances,9 cases with diabetes,3 cases with pancreatic pseudocyst and 2 cases with MOF.Conclusion The systemic nursing is helpful to recovery,which can help to decrease the mortality and improve the prognosis.
