金蒸气激光光动力学疗法治疗上消化道恶性肿瘤的临床研究

目的研究金蒸气激光光动力学疗法治疗上消化道癌的近期疗效及毒副反应。方法临床各期上消化道癌患者67例,男性49例,女性18例,平均年龄71.04岁。应用金蒸气激光器作为治疗光源,激光波长627.8nm,功率密度200~400mW/cm,能量密度100~300J/cm。光敏剂为血卟啉衍生物,按5mg/kg于照光前12~24h静脉滴注。结果完成治疗的67例患者中,完全效应(CR)5例(7.5%),明显效应(PR)40例(59.7%),稍有效应(MR)16例(23.9%),无效(NR)6例(9.0%)。总有效率(CR PR)为67.2%。所有病例均未出现严重毒副反应。结论金蒸气激光光动力学疗法治疗临床各期上消化道癌疗效肯定,毒副反应轻,耐受性好,是一种较好的姑息性治疗手段。

Clinical study of gold vapor laser in photodynamic therapy for upper gastrointestinal tract carcinomas

Objective:To evaluate the clinical efficacy and adverse effects of Gold Vapor Laser in photodynamic therapy for the upper gastrointestinal tract cancers. Methods:Sixty-seven patients with upper gastrointestinal cancers in different clinical stage was treated with photodynamic therapy. Gold Vapor Laser was used as light source. Power density in irradiating was 200 to 400mW/cm and energy density was 200 to 300J/cm. Haematoporphyrin derivative (HPD) was used as photosensitizer, which was given in a dose of 5mg/kg intravenously 12-24 hours before irradiation. Results:Evaluation of the 67 patients' therapeutic effect showed that 5 patients(7.5%) had a complete response(CR), 40 patients (59.7%) had a partial response(PR), 16 patients (23.9%) had a mild response (MR), and 6 patients (9.0%) had no response(NR). The total response rate(CR PR) was 67.2%. No patients had severe adverse effect in the group. Conclusion:Photodynamic therapy is an effective and safe palliative modality to upper gastrointestinal tract carcinomas.
