社区综合心理干预对老年痴呆症的疗效观察

摘要：目的研究社区综合心理干预对老年痴呆症患者的治疗效果。方法对58例经专科医院确诊后转入社区康复的老年痴呆症患者随机分为干预组与对照组，其中干预组30例，对照组28例。在两组原有药物治疗不变且各方面条件无显著性差异的前提下，对干预组进行社区综合心理干预，于干预前后应用长谷川智力表（HDS）、日常生活活动能力量表（ADL）、简明精神状态量表（MMSE）和生活质量量表（QOL—100）进行测试。结果两组患者在干预前HDS、ADL和MMSE测定的结果差异无显著性。干预2年后，干预组的HDS、ADL和MMSE评分分别为7．93±3．72、52．84±15．45、22．89±7．31，对照组分别为5．39±2．91、56．22±20．76、18．89±6．49，干预组病情较对照组改善，差异有显著性（P〈0．05）；干预组QQL—100评分较干预前明显好转（P〈0．05）。结论社区综合心理干预能改善老年痴呆症患者的病情。

The Effect of Community Comprehensive Psychological Intervention on Dementia

Objective To know the effect of comprehensive psychotherapy ancl intervention on dementia in community in Xining city, Qinghai Province. Methods From 2006 to 2008, 58 patients with dementia were divided into two groups Intervention group （n=30） and control group （n =28）] by random digits table. The intervention group was treated with extra comprehensive psychological intervention based on the general medication treatment as the control group. All subjects were evaluated by scales of ADL MMSE, QOL-100 before and after the study. Results The scores of HDS, ADL and MMSE between two groups before the treatment were no significantly different （P〉0.05）. But after 2 years treatment, the scores of HDS, ADL and MMSE in intervention group （7.93±3.72, 52.84±15.45, 22.89±7.31, respectively） were higher than those in the control group （5.39±91, 56.22±20.76, 18.89±6.49） （all P〈0.05）, and the score of QQL-100 in the intervention group was significantly improved after the intervention （P〈0.05）. Conclusion The community comprehensive psychological intervention might play a significant role on the improvement of dementia.
