新鲜羊膜移植治疗眼表疾病的临床观察

摘    要:目的探讨新鲜羊膜移植治疗眼表疾病的可行性并对其疗效进行评价。方法选择本院就诊的翼状胬肉患者56例61眼、广泛眼睑球粘连患者16例18眼、急性化学伤碱烧伤患者5例9眼、结膜乳头状瘤患者3例6眼共80例94眼,分别进行羊膜移植术。术后观察患者视力,裂隙灯检查角膜上皮修复情况,观察羊膜成活情况及新生血管长入情况。随访观察3~24个月。结果94眼均未发生新鲜羊膜植片的急性排斥反应和感染现象。急性严重化学伤患者5例均抢救成功,视力有不同程度提高;睑球粘连患者16例18眼均恢复了眼球运动并提高了视力;56例61眼胬肉患者有4眼复发,复发率为7%。结论新鲜羊膜移植治疗胬肉能有效地降低复发率,治疗化学伤能减轻炎症、减少新生血管形成,减少并发症,为眼表重建提供了一种有效的生物材料。

Abstract:Objective It is to approach the feasibility and effect of treating ocular surface disease with fresh amnion.Methods 80 cases(94 eyes) of ocular surface diseases,including 56 cases(61 eyes) of pterygium,16 cases(18 eyes) of total symblepharon,5 cases(9 eyes) of acute alkali burn and 3 cases(6 eyes) of papilloma of conjunctiva,underwent amnion transplantation.Visual acuity,corneal epithelial rehabilitation under slit lamp,amnion and new vessels growth were observed during the follow up of 3~24 month.Results No acute rejection or infection occurred in all the cases. The 5 cases of acute alkali burn were successfully salvaged and their visual acuities improved to various degrees.Ocular movements were regained in the 16 cases of total symblepharon,and their visual acuities improved too.Among the 61 cases of pterygium,recurrence appeared in 4 cases(7%).Conclusion Fresh amnion can lower the recurrent rate in treating ocular surface disease,inhibit inflammation,and reduce neovascularization and complications in treating chemical injury,thus offers an effective biological material for ocular surface reconstruction.
