经侧脑室后角灌洗与侧脑室前角穿刺引流对重症脑室出血疗效的比较研究

目的　探讨治疗脑室出血的有效治疗方法。方法　采用侧脑室后角穿刺引流、大量生理盐水灌洗治疗脑室出血 3 6例 ,并与同期单纯侧脑室前角穿刺引流治疗脑室出血 3 3例进行比较。结果　观察组 3 6例 ,显效 2 4例 ,有效 6例 ,无效 6例 ,死亡 6例 ,死亡率 16 7% ,有效率 83 .3 % ,对照组 3 3例 ,显效 18例 ,有效 1例 ,无效 6例 ,死亡 8例 ,死亡率 2 4.2 % ,有效率 5 7.6%。两组比较具有显著差异 ,P <0 .0 5。结论　侧脑室后角穿刺、灌洗治疗脑室出血简便易行、有效。

Comparison of the Effects between Lavage via Posterior Horn and Puncture via Anterior Horn of Lateral Ventricle in the Therapy of Severe Ventricular Hemorrhage

Objective To study the effective way to the therapy of brain ventricular hemorrhage. Methods Thirty-six patients (observing group) with severe brain ventricular hemorrhage were treated with puncture and drainage via posterior horn of lateral ventricle plus lavage with a large amount of physiological saline solution, and then compared with 33 patients (control group) with the same malady who were treated simply with puncture plus drainage via anterior horn. Results In observing group, there were 24 and 6 patients resulting respectively in clear and so-so effect (effective rate 83.3%), and 6 ones in no effect. The other 6 patients (16.7%) died. In control group, 18 patients were clear effective, and one was so-so (effective rate 57.6%). There were 6 and 8 patients respectively resulting in no effect and death (death rate 24.2%). There was significant difference between the two groups (P<0.05). Conclusion Puncture via posterior horn of lateral ventricle combined with lavage is a simple, convenient and effective way to the therapy of brain ventricular hemorrhage.
