老年低位结直肠癌并发急性肠梗阻外科治疗分析

目的观察老年低位结直肠癌并发急性肠梗阻的外科治疗效果。方法回顾性分析78例结直肠癌并发急性肠梗阻老年患者的临床资料。结果Ⅰ期右半结肠切除吻合术23例,Ⅰ期左半结肠切除吻合术28例,乙状结肠单腔造口术12例,Mile’s术13例,直肠癌前侧切除术2例。术后感染5例,其中包括切口感染2例、泌尿系感染2例、肺部感染1例;吻合口瘘3例;多器官功能衰竭2例,术后并发症发生率为12.82%。其中76例患者痊愈或好转后出院占97.44%,死亡2例占2.56%。结论老年低位结直肠癌并发急性肠梗阻的治疗宜首选手术治疗方法,根据具体病情选择合适的手术方式,并做好围术期处理,可有效减少并发症,降低病死率。

Analysis of surgical treatment of elderly low colorectal cancer complicated with acute intestinal obstruction

Objective To observe the surgical effect of elderly low colorectal cancer complicated with acute intestinal obstruction.Methods Retrospectively analyed the clinical data of 78 cases of elderly low colorectal cancer complicated with acute intestinal obstruction.Results Right colon resection and anastomosis in 23 cases,Left colectomy and anastomosis in 28 cases,Hartmann stoma in 12 cases,Mile ＇s operation in 13 cases,Dixon in 2 cases.Postoperative infection occurred in 5 cases,including 2 cases of wound infection,urinary tract infection in 2 cases,1 cases of pulmonary infection;anastomotic fistula in 3 cases;2 cases of multiple organ function failure,the incidence of postoperative complications was 12.82%.76 patients were cured or improved after discharge,accounted for 97.44%,2 cases died,accounted for 2.56%.Conclusion Operation treatment should be the first choice of elderly low colorectal cancer complicated with acute obstruction.According to the specific condition,choose suitable operation method,and do a good job in peri operation period processing,which can effectively reduce complications,reduce mortality.
