摘    要:【目的】探讨超敏C-反应蛋白(hs-CRP)与尿毒症维持性血液透析(maintenance hemodialysis,MHD)患者贫血、营养状态之间的关系。【方法】根据hs-CRP水平将58例MHD患者分为升高组(A组:hs-CRP≥3 mg/L)及正常组(B组:hs-CRP〈3 mg/L)进行对照研究。检测患者的超敏C-反应蛋白(hs-CRP)、各项血清学营养指标,同时记录患者每周重组人促红细胞生成素(rHuEPO)用量。【结果】A组Hb、ALB均低于B组Hb、ALB,两组比较均有显著性差异(P〈0.01);直线相关性分析结果,血hs-CRP与Hb、Alb呈显著负相关,与每周EPO用量呈正相关。两组间年龄、性别、体重及Kt/V指标未见显著差异。【结论】尿毒症MHD患者hs-CRP水平普遍增高,同时超敏C-反应蛋白作为慢性炎症反应状态最具代表性的标志之一,可以用来评价MHD患者的营养状况,并可作为间接判断维持性血液透析患者预后的指标。

Abstract:【Objective】To discussthe relationship between high-sensitivity C-reactive Protein with anemia,nutritional status in the maintenance hemodialysis patients.【Methods】Fifty-eight maintenance hemodialysis patients were divided into high group(A group: hs-CRP≥3 mg/L) and normal group(B group:hs-CRP〈3 mg/L)according to the levels of hs-CRP.The relationships betweeen hs-CRP and hemoglobin,serum nutritional indicators were observed.The dose of rHuEPO used per week was recorded.【Results】The levels of Hb,ALB in A group,were lower than B group,and were significantly different(P0.01).Linear correlation analysis of the results of hs-CRP with Hb,Alb were significantly negatively correlated;EPO dose per week used;were positively correlated.There were no significant differences in indicators between the two groups of age,sex,weight,and Kt/V.【Conclusions】Hs-CRP level is generally higher in MHD patients(The normal reference value〈3 mg/L).At the same time,high-sensitivity C-reactive protein as one of the most representative symbol in the state of chronic inflammation,which can be used to evaluate the maintenance hemodialysis patients with nutritional status and may be served as an indirect predictor in determining the prognosis of maintenance hemodialysis patients.
