412例多发伤后全身炎症反应综合征临床分析

目的 :分析多发伤后全身炎症反应综合征 (SIRS)的发生率和病死率以及SIRS与休克的关系。方法 :多发伤 4 12例 ,分为SIRS组和非SIRS组 ,比较两组的病死率 ,并进行SIRS评分以及按休克严重程度分组 ,分析多发伤后SIRS与休克的关系及多发伤病死率与SIRS评分的关系。结果 :多发伤后SIRS发生率为 87 6 % ,SIRS组病死率明显高于非SIRS组 (P <0 0 5 )。随休克严重程度的增加 ,SIRS发生率明显升高 (r=1,P <0 0 0 1) ,同时多发伤病死率也随着SIRS分值的增加而增高 (P <0 0 0 5 )。结论 :大多数多发伤患者发生了SIRS,发生SIRS后病死率较高 ,SIRS的发生率与休克严重程度成正相关。积极抗休克治疗及早期进行脏器功能支持应是防治SIRS的有力的措施。

Clinical Study of Systemic Inflammatory Response Syndrome from 412 Cases of Multiple Trauma Patients

Objective:The study was designed to explore incidence of systemic inflammatory response syndrome(SIRS)and the correlation of SIRS with shock after multiple trauma.Methods:four hundred and twelve patients were divided into systemic inflammatory response syndrome(SIRS)group and non-SIRS group.The mortality rates of SIRS group were compared with that of non-SIRS group.The relationship between SIRS and shock was observed.Results:SIRS occurred in 87.6% multiple trauma patients.The mortality rates of SIRS group were significantly high compared with those of non-SIRS group(P<0.05).When the severity of shock increased,the morbidity rates of SIRS increased(r=1,P<0.001).Conclusion:Most of the patients with multiple trauma suffered from SIRS.The mortality rate of SIRS was high in multiple trauma patients.The severity of shock was positively Corelated with SIRS.The effective strategy for prevention and treatment of SIRS would be remedying shock timely and supporting the functions of the organs early.
