老年高血压左心室肥厚与动态脉压相关性的临床观察

目的:探讨老年高血压患者左心室肥厚(LVH)与动态脉压和主动脉根部内径(AOD)的关系。方法:应用24小时动态血压监测和超声心动图检查,测量并计算88例高血压患者24小时平均动态脉压,左室重量指数(LVMI)及主动脉根部内径。入选患者均进行病史采集,计算体重指数(BMI)、查空腹血糖(FBS)、血清甘油三脂(TG)、血清总胆固醇(TC)、高密度脂蛋白-胆固醇(HDL-C)。分左室肥厚组和无左室肥厚组,分别进行统计学比较。结果:24小时动态脉压,AOD及LVMI在高血压左心室厚组和无左室肥厚组之间的差异均有显著性(P<0.01);年龄及高血压病程在两组间差异有显著性(P<0.01)。结论:脉压增加和AOD扩大是老年高血压伴左心室肥厚患者的共同危险因素,与年龄及高血压病程相关。

Association between left ventricular hypertrophy and ambulatory pulse pressure in senile patients with essential hypertension

Objective:To explore the relationship between left ventricular hypertrophy(LVH) and ambulatory pulse pressare(APP) and arotic root dimension(AOD) in senile patients with essential hypertension.Methods:88 senile patients with essential hypertension were observed in APP,left ventricular mass index(LVMI) and AOD with ambulatory blood pressure monitoring and echocardiography.Their clinical history was obtained.And body mass index(BMI),fasting blood sugar(FBS),triglyceride(TG),totle chotesterol(TC),highdensity lipoprotein-chotesterol(HDL-C) and lowdensity lipoprotein-chotesterol(LDL-C) were examined.The patients were devided into 2 groups,LVH group and non-LVH group.Their results were observed and compared respectively.Results:24-hour ambulatory pulse pressure,AOD and LVMI were significantly different between LVH group and non-LVH group(P<0.01).They were significantly correlated with age,history of hypertension(P<0.01).Conclusion:The increase in pulse pressure and augment in AOD were the important determinant of LVH in senile patients with essential hypertension.And they were corretated with age,history of esstential hypertension.
