64排螺旋CT联合超声内镜对胃癌术前T分期的诊断价值

目的评价64排螺旋CT（MSCT）联合超声内镜（EUS）在胃癌术前T分期的价值。方法经手术证实的50例胃癌患者，术前均采用64排螺旋CT扫描，同期行EUS检查，综合MSCT及EUS的结果进行术前T分期，并与术后病理对照，评价准确率。结果50例胃癌MSCT术前T分期总体准确率为80％，其中早期胃癌准确率为62．5％，进展期胃癌准确率为83．3％。EUS术前T分期总体准确率为74％，其中早期胃癌准确率为75％，进展期胃癌准确率为73．8％。MSCT联合EUS后对术前胃癌T分期的准确率明显提高，总体准确率为86％，其中早期胃癌准确率为75％，进展期胃癌准确率为88．1％。结论MSCT联合EUS对早期、进展期等各期胃癌的术前T分期准确率明显提高。

The Value of 64-Slice Spiral CT Combined with Endosonography in the T Staging Diagnosis of Gastric Cancer

Objective To evaluate the value of 64-slice Spiral CT（MSCT） combined with endosonography （EUS） in the T staging diagnosis of gastric cancer.Methods 50 patients with gastric cancer underwent the T staging by MSCT and EUS before operation.The results were strictly compared with surgical pathology, to evaluate the accuracy of the staging results combined with MSCT and EUS.Results The total accuracy of T staging by MSCT is 80%,62.5% to early gastric cancer and 83.3% to advanced gastric cancer.The total accuracy ofT staging by EUS is 74%,75% to early gastric cancer and 73.8% to advanced gastric cancer respectively.The total accuracy of T staging combined with MSCT and EUS raise significantly with a total accuracy of 86%.The detection rate to early gastric cancer is 75% and 88.1% to advanced gastric cancer.Conclusion The combination of MSCT and EUS can raise the accuracy of T staging of gastric cancer significantly in all staging,both early gastric cancer and advanced gastric cancer.
