经伤椎椎体内植骨结合内固定治疗胸腰椎骨折

[目的]总结经伤椎椎体内植骨结合内固定治疗胸腰椎骨折的临床疗效。[方法]经伤椎椎弓根椎体内植骨结合内固定治疗无神经功能损害、载荷分享评分≥7分的胸腰椎A型、部分B型骨折86例,并观察患者椎体术前术后的椎体矢状位后凸角。[结果]随访时间12∽36个月,平均（24.00±6.00）个月。86例患者术后椎体后凸角均显著恢复,但有8例患者术后9个月发现椎体后凸角明显丢失平均12.5°（占9.3%）。[结论]经伤椎椎体内植骨内固定术可以恢复脊柱生理弯曲及伤椎前缘高度,又能重建椎体的骨密度,有效防止日后矫正度的丢失,在严格掌握手术指征前提下,是治疗胸腰椎骨折的[方法]之一。

Transvertebral Bone Grafting Combined with Internal Fixation for the Treatment of Thoracolumbar Fracture

[Objective] To investigate the effect of transvertebral bone grafting combined internal fixation in the treatment of thoracolumbar fracture. [Methods] We selected 86 cases of type A and partial type B thoracolumbar fracture without neurological deficit, and load sharing classification ≥7. Compare preoperative and postoperative kyphosis angle. [Results] The fol ow-up duration was 12∽36 months and 24 months in average. The kyphosis angle was retrieved,but there were eight cases（9.3%） in which kyphosis angle was lost 12.5＆#176;in average after nine months. [Conclusion] Transvertebral bone grafting combined with internal fixation restored the vertebral curve and vertebral anterior height, retrieved vertebral BMD and height ,and prevented the failure of reclamation. Under the indications, transvertebral bone grafting combined with internal fixation was effective for thoracolumbar fracture.
