Understanding the essential characteristics of the five modes helps provide reference information for the studies on DM program management conducted by community centers for health services. An appropriate management model of integrated DM prevention and cure for communities will better meet the needs of DM patients, have a long-term and continuous surveillance and control of health conditions of DIM patients, and promote cognition rate, cure rate, and control of blood glucose in DM patients of the communities.
