无创与有创正压通气救治溺水后并发急性呼吸衰竭疗效比较

 目的 观察无创正压通气抢救溺水所致的急性呼吸衰竭的疗效,并与有创通气比较.方法 选择25例溺水所致的急性呼吸衰竭患者,分为无创组(13例)与有创组(12例),常规治疗相同,分别实施机械通气,监测呼吸频率、心率、动脉血气分析等.结果 25例全部救治成功.两组治疗0.5 h后临床症状、体征明显好转,血气分析各指标、呼吸频率、心率均好转,与通气前比较差异有统计学意义(P<0.05);1 h后无创组与有创组同时段各监测指标比较差异无统计学意义(P>0.05);无创组与有创组机械通气压力差异无统计学意义(P>0.05),无创组机械通气时间短于有创组(P<0.05),住院时间、医疗费用也较有创组明显减少(P<0.01).结论 无创正压通气在救治溺水导致的病情较轻的急性呼吸衰竭方面与有创通气一样安全有效.

A comparison of noninvasive and invasive positive pressure ventilations for treatment of drowning patients with acute respiratory failure
