改良宫颈缝合成形术在冷刀锥切术中的应用

目的:探讨改良宫颈缝合成形术(改良Sturmdorf缝合术)在冷刀锥切术中的止血效果。方法:1999年至2005年在北京大学人民医院行宫颈冷刀锥切术88例,其中采用改良宫颈缝合成形术止血38例(缝合组),采用电凝止血和碘仿纱布压迫法止血50例(非缝合组)。术前诊断两组间无明显差异。比较两组手术时间、术中出血、术后出血、术后病理和术后住院时间。结果:缝合组平均手术时间35分钟,明显短于非缝合组42分钟(P=0.033);平均术中出血量73 ml,明显少于非缝合组113 ml(P=0.017);平均术后出血量19 ml,少于非缝合组66 ml(P=0.0003);平均术后住院时间6.4天,少于非缝合组8.4天(P=0.019)。两组术后诊断无明显差异。结论:改良宫颈缝合成形术不影响冷刀锥切治疗效果,能有效进行创面止血,缩短术后住院时间。

Hemostasis Effect of Plastic Suture of Cervix on Cold Knife Conization

Objective: To explore the hemostasis effect of modified Sturmdorf plastic suture of cervix on wound of cold knife conization. Methods: We retrospectively reviewed 88 women who underwent cold knife conization in the Department of Gynecology of Peking University People's Hospital from 1999 to 2005.According to their hemostasis method,they were divided into suture group(n =38) with modified Sturmdorf plastic suture and the non-suture group(n =50) with electric coagulation or iodoform gauze compressura.The operative time,intraoperative and postoperative blood loss,postoperative pathology and postoperative hospital staying were compared between two groups. Results: The operative time of suture group was shorter than non-suture group(35 vs.42 min,P =0.033).Both intraoperative(73 vs.113 ml,P =0.017) and postoperative blood loss(19 vs.66 ml,P =0.0003) of the suture group were less than the non-suture group.Postoperative hospital staying of the suture group is shorter than non-suture group(6.4 vs.8.4 d,P =0.019).There was no difference of postoperative pathological diagnosis between two groups. Conclusions: The modified Sturmdorf plastic suture of cervix presents effective hemostasis in cold knife conization,leading to less blood loss and shorter postoperative hospital staying.
