IL-6及其受体和凋亡相关基因在胃癌中的表达

目的:探讨正常胃粘膜、癌前病变和胃癌组织中的白细胞介素6(IL-6)及其受体、Fas、FasL、Bcl-2的表达规律及临床意义.方法:应用原位杂交技术检测不同胃粘膜病变标本中IL-6及其受体mRNA的表达情况;应用免疫组织化学方法对不同胃粘膜病变标本中Fas、FasL和Bcl-2蛋白的表达情况进行检测.结果:IL-6及其受体在正常胃粘膜中的表达明显低于癌前病变(P＜0.05);在癌前病变中的表达明显低于早期胃癌(P＜0.05);而早期胃癌中IL-6及其受体的表达与进展期胃癌相比无明显差异(P＞0.05).癌变粘膜中Fas的表达明显低于正常粘膜和癌前病变(P＜0.05),正常粘膜与癌前病变、癌前病变与癌变粘膜之间FasL的阳性表达率存在着明显统计学差异(P＜0.05).正常粘膜Bcl-2的阳性表达率明显低于其余粘膜(P＜0.05).结论:IL-6是反应胃癌发生发展的一个重要指标;临床上,对Fas、FasL、Bcl-2进行相应的调节将可能成为治疗胃癌的有效辅助手段之一.

STUDY ON THE EXPRESSION OF IL-6, IL-6R AND APOPTOSIS-ASSOCIATED GENES IN GASTRIC CARCINOMA

Objective: To study the expression of IL-6 and its receptor(IL-6R), Fas, FasL and Bcl-2 in normal gastric mucosas, precancerous lesion, gastric carcinoma,and to explore their clinical significance. Methods: The expression of IL-6 and IL-6R mRNA were detected by means of In situ hybridization(ISH), the expression of Fas,FasL and Bcl-2 were detected by means of immunohistochemical technique. Results: The positive rates of IL-6 and IL-6R were significantly different between normal gastric mucosas and precancerous lesion(P<0.05),and there were significant difference between precancerous lesion and early gastric carcinoma(P<0.05). The levels of Fas expression in gastric carcinoma were obviously lower than those in both normal mucosas and precancerous lesion (P<0.05). Significant difference of FasL expression were found both between normal mucosas and precancerous lesion and between precancerous lesion and gastric carcinoma(P<0.05). There was significant difference for expression of Bcl-2 between normal gastric mucosas and both precancerous and gastric carcinoma(P<0.05). Conclusions: IL-6 and its receptor are two important indexes that reflect the occurrence and development of gastric carcinoma.In clinical,it would be a effective means to heal gastric carcinoma by adjusting Fas,FasL and Bcl-2.
