低增生性急性白血病8例报道

本文报道一种少见类型的低增生性急性白血病。其诊断根据为:1.多次多部位骨髓穿刺有核细胞增生低下;2.骨髓中原始加早(幼)细胞在30%以上,且有白血病细胞形态学改变;3.骨髓病理活检造血细胞减少。目前,该病尚无满意治疗方法,国内外在近10多年的报道中疗效均较差。我们对5例HAL应用逐步增强化疗法,均取得了完缓解(CR),CR期最长已达15个月,目前仍处于CR中,疗效满意。本文着重讨论了逐步增强化疗法的原则、方法及临床应用价值。

A Report of 8 Cases of hypoplastic acute leukemia

In this article, an uncommon type of acute leukemia, i.e. hypoplastic acute leukemia(HAL) is reported. Its diagnostic criteria are: (1)multi-sited and repeated marrow pu-ncture demonstrated that nucleate cell proliferation is low; (2) myeloblasts and progranu-locytes in the bone marrow is over 30 per cent, and the morphological change of leuke-mia is noticed: (3) bone marrow biopsy shows that hematopoietic cells decreased. So farthere has been no satisfied therapy for this disease. During the last ten years, most reportsshowed poor curative effect at home and abroad. We have achieved satisfactory curativeeffect through gradual enhancing chemotherapy. 8 cases of HAL in total in our hospitalwere admitted and treated from 1981 to 1986. We used this chemotherapy to 5 pa-tients with HAL, and complete remission (CR) was found in all of them. The longestCR time has reached 15 months, and now this patient has still been in CR. The princi-ple, method and value of the clinical application of gradual enhancing chemotherapy arediscussed in this article.
