脑膜瘤硬脑膜侵袭的临床研究

目的：探讨脑膜瘤侵袭硬脑膜MRI表现和组织病理的关系及其临床意义。方法：选取36例脑膜瘤MRI成像，外科手术切除肿瘤连同附近硬脑膜进行组织病理检查，作MRI病理对照观察研究。结果：全部36例脑膜瘤中有26例MRI表现脑表面硬脑膜线样增强带，其中21例MRI表现不连续性硬脑膜MRI表现增强带，组织病理检查提示有肿瘤侵袭硬膜；其余5例硬脑膜呈连续且厚度均匀一致，组织病理表现为血管扩张和结缔组织增生。结论：脑膜瘤硬脑膜MRI表现增强带较常见，连续且均匀一致的硬脑膜增强带提示反应性增生，不连续的硬脑膜增强带则往往有肿瘤侵袭，手术时应予充分切除。

Clinical study on dural invasion in meningioma

Objective To investigate the correlation of MRI and histopathological findings and its clinical implications in meningioma with dural invasion. Methods A surgical, histopathological, and MRI study was performed in 36 patients with meningioma. Results Among of the 36 meningioma patients, 26 cases had lined-enhanced dura disclosed by MRI. Histopathological examination found that dura with a break in the continuity of enhancement was invaded by the tumor in 21cases. The other 5 cases had histopathological findings of vascular extension and hyperplasia of connective tissues. Conclusion Enhanced dura in meningioma often occurred. A fairly uniform enhanced dura adjancent to a meningioma correlated with a dural inflammatory reaction, while discontinuous enhancing dura indicated dural invasion which should be resected completely.
