剖宫产与顺产对早产儿血气、电解质、肾功能及心肌酶的影响研究

摘    要:目的:探讨剖宫产和顺产对早产儿血气、电解质、肾功能及心肌酶指标的影响.方法:回顾性分析2013年8月-2015年10月在我院出生的早产儿的临床资料.根据其出生方式分为剖宫产组和顺产组,比较两组早产儿出生后Apgar评分、血气、电解质、肾功能及心肌酶指标的差异,观察两组早产儿并发症发生率.结果:顺产组早产儿出生后1min和5min的Apgar评分分别为9.82±0.54分和9.95±0.62分,与剖宫产组早产儿比较无明显差别;两组早产儿Hb、PLT、SpO2、P ETCO2和CVP水平均无明显差别;两组早产儿血钠、血氯、尿素氮、肌酐和CK-MB水平均无明显差别;顺产组吸入性肺炎和颅内出血发生例数分别为4例和1例,剖宫产组吸入性肺炎和颅内出血发生例数分别为1例和0例,两组早产儿并发症的发生率无明显差别.结论:剖宫产和顺产对早产儿血气、电解质、肾功能及心肌酶指标均无明显影响,临床可根据孕妇具体情况选取合适的分娩方式.

Abstract:Objective To investigate the effect of cesarean section and vaginal delivery on blood gas, electrolyte and renal function in preterm infants and myocardial enzymes. Methods The clinical data of preterm infants born in our hospital from August 2013 to October 2015 were retrospectively analyzed. According to the mode of delivery were divided into cesarean section and normal labor group. The differences of Apgar score, blood gas, electrolyte, renal function and myocardial enzyme in-dexes were compared between the two groups after birth, and the incidence of complications in the two groups were observed. Results Spontaneous preterm infants born after 1 min and 5 min Apgar score respectively 9.82 + 0.54 and 9.95 + 0.62 points, and caesarean birth infants in group a compared no significant difference; The levels of Hb, PLT, SpO2, ETCO2, P and CVP were not significantly different in the two groups. There was no significant difference between the two groups in the levels of serum sodium, serum chlorine, urea nitrogen, creatinine and CK-MB; Delivery group inhaled pneumonia and intracranial hem-orrhage. There were 4 cases and 1 case, cesarean section group inhaled pneumonia and intracranial hemorrhage cases, respec-tively, the number of 1 and 0, the two groups of premature infants with complications occurred rate had no significant difference. Conclusion Cesarean section and vaginal delivery on blood gas of premature infants, electrolytes, renal function and myocardial enzyme index had no obvious effect, clinical can select the appropriate mode of delivery according to the specific situation of the pregnant women.
