血清白蛋白对结肠癌患者手术后预后的影响

目的:研究结肠癌患者术前血清白蛋白的临床意义以及其对术后预后的影响.方法:回顾性分析2008年3月~2012年12月期间我院收治的326例结肠癌患者的临床资料.以血清白蛋白35g/L为界限,将患者分为正常组及低白蛋白组.分析血清白蛋白水平与患者临床病理特征的相关性,并用Cox回归模型分析结肠癌患者预后的影响因素.结果:低白蛋白血症与结肠癌肿瘤部位、分化程度、TNM分期及浸润深度相关,并随着分化程度、TNM分期及浸润深度的进展其发生率明显增高;TNM分期、肿瘤浸润深度以及术前低白蛋白水平为结肠癌患者的独立预后因素;术前血清白蛋白水平正常患者3、5年生存率分别为75.6%、63.7%;低白蛋白血症患者3、5年生存率分别为61.4%、53.9%,差异比较有统计学意义.结论:术前血清白蛋白水平与结肠癌肿瘤部位、分化程度、TNM分期及浸润深度相关,也是结肠癌患者术后的独立预后因素.

Influence of serum albumin to prognosis of colonic neoplasms after surgery

Objective To explore the Influence of serum albumin to prognosis of colonic neoplasms after surgery. Methods Retrospectively analyzed the clinic data of 326 patients with colonic neoplasms from March 2008 to December 2012. And the were divided into normal group and hypoalbuminemie according to the definition of serum albumin<35g/L. Comapred the asso-ciation between clinicopathological features and preoperative serum albumin level. Multivariate Cox model was used to anaylze the prognostic factors. Results Hypoalbuminemia had significantly incidence with colon tumor location, tumor differentiation, TNM stage and invasion depth, the degree of differentiation, TNM stage and depth of invasion of its progress; before TNM stage, invasive depth and hypoalbuminemia is an independent prognostic factor in patients with colon cancer; 3, 5-year survival rates of patients with preoperative serum albumin normal levels were 75.6%, 63.7%; and 3, 5-year survival rates of hypoalbuminemia patients were 61.4%, 53.9%, the difference was statistically significant . Conclusion Preoperative serum albumin levels has correlationwith colon tumor location, tumor differentiation, TNM stage and depth of invasion, and also is an independent prog-nostic factor of colon cancer.
