摘    要:目的 :探讨不同呼吸机在新生儿呼吸窘迫综合征早产患儿中的临床疗效.方法 :利用随机选取法选取我院2014~2016年120例新生儿呼吸窘迫综合征患儿并分为机械通气组、气道持续正压通气组和经鼻持续气道正压通气组各40例.通过对比分析3种不同呼吸机在使用前后对新生儿呼吸窘迫综合征早产患儿的动脉血气指标、通气时间、住院时间和肺部感染和鼻粘膜损伤程度的情况.结果 :从动脉血气指标上看,经鼻持续气道正压通气组在使用呼吸机后其动脉血二氧化碳分压均比其他两组的低,动脉血氧分压均比其他两组的高,但随着使用时间的推迟,3组患儿的血气指标并未存在明显差异;经鼻持续气道正压通气组使用呼吸机后患儿并发症发生率明显比其他两组低,且其住院时间和医疗费用比其他两组少.结论 :3种不同呼吸机的对比得出经鼻持续气道正压通气呼吸机具有更高的治疗率和较低的并发症发生率,且能缩短患儿住院时间,减轻患儿家庭的经济负担,临床上更值得推广.

Abstract:Objective To compare the clinical effectson treatment of neonatal respiratory distress syndrome by different breathing machines. Methods 120 casesdiagnosed asneonatal respiratory distress syndrome were randomly selected in our hospital during 2014 to 2016and divided into mechanical ventilation, continue positive airway pressure, nasalcontinue positive airway pressure, with each group 40 cases. Before and after treatment, arterial blood gas index, duration of ventilation, hospitali-zation time, pulmonary infection and the injury of nasal mucosa of three groups were compared. Results On the view of arterial blood gas index, the PaCO2were lower, PaO2was higherof the nasal continuous positive airway pressure groupthan the other two groups after usingbreathing machines, but with time went by, arterial blood gas index of 3 groups had no obvious difference ;the complications of the nasal continuous positive airway pressure group was significantly lower than the other two groups, and the hospitalization time and medical costs were less than the other two groups. Conclusion Compared with 3 different breathing machines, nasalcontinue positive airway pressure has higher efficiency and lower complications, shortens hospitalization time and reduces family's economic burden, so it should be vigorously promoted in clinical.
