106例食管癌术前营养风险及其危险因素的分析

摘    要:目的:调查可手术食管癌术前营养风险与营养不足发生率,并分析营养风险及营养不足的危险因素,为降低营养风险的发生提供依据.方法:使用营养风险筛查2002(NRS2002)结合体质指数(body mass index,BMI)对住院进行手术治疗的106例食管癌患者进行营养风险调查,并分析营养不足发生率.运用单因素分析术前营养风险状况与患者基本情况、生活习惯、饮食习惯之间的关系,运用多因素Logistic回归分析术前营养风险的独立危险因素.结果:所有106例患者中,具有营养风险者(NRS2002≥3)84例,营养风险发生率79.25%(84/106);营养不足者(BMI<18.5)13例,营养不足发生率为12.26%(13/106).年龄因素、年龄是否≥65岁、职业因素、饮用水源是发生营养风险的影响因素,饮食习惯中的食用干果干菜是发生营养不足的影响因素.经Logistic回归分析,年龄因素、饮用水源是营养风险发生的独立危险因素.结论:食管癌术前营养风险与营养不足发生率高,术前营养支持治疗还有待进一步改善.

Abstract:Objective To explore the occurrence rate of the preoperative nutritional risk and the malnutrition rate with the esophagus cancer. Analyze the influence factors of the nutritional risk and to provide basis of clinical nutrition support.Meth-ods Observe the occurrence rate of the nutrition risk and the malnutrition rate, use independent test and χ2 test to analyze the factor that related to nutrition risk, use logistic regression analysis to find the independent factor of the nutrition risk Results In all the 106 patients there are 84 of them NRS2002 rating≥3, and nutritional risk rate was 79.25% (84/106).13 cases were BMI<18.5kg/㎡, malnutrition rate was 12.26% (13/106). Older than65、professional factor、source of drinking water were the domain factors with nutrition risk. The dietary factor of eating dried fruits and vegetables (t value were 2.244) were the domain factor with malnutrition . Logistic regression analysis show the age, Drinking water source is an independent risk factor for the occurrence of nutritional risk.Conclusion High occurrence rate of nutrition risk will appear to patients who underwent esopha-gus cancer surgery, the clinician should take more effective nutrition support to those patients who under nutrition risk.
