24小时pH监测比较西沙必利与雷尼替丁的抗胃食管反流效果

用２４小时ｐＨ动态监测分别观察西沙必利（普瑞博思）和雷尼替丁治疗２１例反流性食管病的抗反流效果，观察两种药物对胃内ｐＨ的影响与抗胃食管反流（ＧＥＲ）作用的相关性。结果表明，西沙必利组和雷尼替丁组治疗后食管酸反流指数均较服药前下降，两组间下降指数比较无显著性差异（Ｐ〉０．０５）。西沙必利组对胃内ｐＨ无影响，雷尼替丁组可降低胃内ｐＨ〈１，ｐＨ〈２和ｐＨ〈３的总时间（％），平均ｐＨ和中位ｐＨ升高，两组比

24-hour Gastric pH Monitoring to Compare the Antireflux Effect of Cisapride with Ranitidine

24-hour pH monitoring was performed in 21 patients with gastroesophageal reflux disease(GERD) when they were treated with cisapride or ranitidine separately. The changes of intragastric pH fluctuation and antireflux effect were compared. Before and after taking cisapride 10mg qid(n = 11)or ranitidine 150mg bid(n = 10)for 7 days, the esophageal and intragastric pH of the patients were monitored by two sensors of monocrystant antimony pH catheter. The results revealed that the reflux index was lower after taking medicine in both groups,but there was no difference between the two groups(P>0.05). Cisapride had no antisecretory action, ranitidine could decrease total duration of intragastric pH< 1,2, and< 3 and increase mean pH and median pH,which were obviously different in two groups(P<0.01). There was no correlation between intrgastric pH fluctuation and antireflux index after medication in both groups( r= -0.436 and -0.341,P>0.05).The data suggested that cisapride and ranitidine had same antireflux action, but their mechanisms were different. Therefore, it is important to properly choice the right medicine for appropriate patients.
