骨原发性间变性大细胞淋巴瘤临床病理观察

目的探讨骨原发性间变性大细胞淋巴瘤（ALCL）的临床病理特点、鉴别诊断及预后。方法观察4例骨原发性ALCL的临床病理和免疫组化特点，随访7～45个月，并复习相关文献。结果骨原发性ALCL多见于中、青年男性。组织形态上，肿瘤细胞弥漫浸润骨组织，细胞高度异型，核分裂象多；免疫组化标记示CD30和EMA（＋），ALK-1和粒酶B（＋）较多。4例患者中，2例分别在随访7个月和11个月时死亡。结论骨原发性ALCL十分罕见，明确诊断依赖于组织形态学观察及免疫组化标记，其预后可能存在个体差异。

Primary bone anaplastic large cell lymphoma: a clinicopathological analysis

Objective To investigate the clinicopathologic characteristics, differential diagnosis and prognosis of primary bone anaphstic huge cell lymphoma. Methods We examined the clinicopathologic and immunohistochemical features in 4 cases of primary boned anaplastic large cell lymphoma, followed up for 7 to 45 months and reviewed the related literatures. Results Primary boned anaplastic large cell lymphoma mainly occured in the male patients with young or middle age. Tumor cells infiltrated bone, exhibited high pleomorphism and a large amount of mitotic figures. Immunohistochemical studies found that all reacted positively with CD30 and EMA, and the cases frequently expressed ALK-1 and granzyme B. Among 4 follow-up patients, 2 eases died within 7 months and 11 months of diagnosis respectively. Conclusions Primary bone anaplastic large cell lymphoma is extremely uncommon. The definite diagnosis should be based on the histopathological and immunohistochemical features to exclude other tumors. The prognosis is possibly heterogeneous.
