眶上锁孔入路显微手术治疗鞍上区肿瘤

目的 介绍眶上锁孔入路显微手术切除鞍上区肿瘤的技术和经验。方法 选择鞍上区10例脑膜瘤和3例垂体腺瘤，经眶上锁孔入路，运用显微外科技术切除肿瘤，对周围结构妥善保护。结果 肿瘤全切除12例（92．3％），大部分切除1例（7．7％）。除1例有短期尿崩外，余病例无术后感染、出血等手术并发症；术后住院时间明显缩短。结论 眶上锁孔入路对脑组织无效暴露少、牵拉轻、损伤小、术后并发症少，提高了手术安全性，有利于预后。

Supraorbital keyhole approach for suprasellar region tumors

Objective To introduce the technique and experience of supraobital keyhole approach for suprasellar region tumors. Methods Ten meningiomas and 3 pituitary adenomas were selected, and supraorbital keyhole approach and microsurgical technique were used to excise tumors with peripheral tissues protected properly. Results Twelve tumors(92.3% )were removed totally, and one tumor (7.7% ) was removed partially. All patients had no complications such as infection and hemorrhage except 1 case who suffered transitory diabetes insipidus, so hospitalization time was obviously shortened. Conclusion Supraobital keyhole approach can minimize the exposing time, decrease the traction injury to the brain. It can also reduce the operative complications, increase safety and favor prognosis.
